Glazier Medical Centre Family Health Organization

The primary health care provider is the first point of contact between a patient and the health care system. Primary care in Ontario strives to provide for comprehensive primary care and to respond to the needs of the whole person, ensure continuity of care, and acute and chronic disease management, as well as health promotion and disease prevention.

Family Physicians who work at Glazier Medical Centre are members of the Glazier Family Health Organization. Here at the Centre, the group of family physicians have committed to make every effort to offer their patients comprehensive primary care. This commitment includes offering patients extended hours for urgent non-emergency issues, working as a group to ensure patients always have access to primary care, providing a wide range of in-office services such as lab work, and extended nursing care for chronic disease management. Because the group works as a team, even if your physician is away or on leave, the other family practitioners can oversee your care.

Patients are “enrolled” in the group to ensure continued accessible quality office care throughout the day and evening and have access to a nurse-staffed Telephone Health Advisory Service. Although not required, enrolling and seeing your physician within the group will ensure that you have full access to this care.
