What is the Ministry of Transport Medical Exam
If you are applying for a Class A, B, C, D, E, or F licence you will need to have a medical review and physical examination by a qualified physician at standard intervals based on age (18-45 every 5 years, 46-64 every 3 years, and 65+ every year). This is for the Ministry of Transport to ensure there are no medical issues impacting your driving ability. The ministry mails drivers medical forms to you 90 days before your licence is due for renewal.

How to prepare for your appointment
Please arrive 5-10 minutes prior to your appointment time to check in.
Parking is free on side streets and $7 for all day parking on site
Bring the following items:
- All medications in their bottles or blister pack
- Glasses worn while driving, if applicable
- Hearing aids, if applicable
- Your medical examination form
What to expect at your appointment
Upon arrival you will check in at Area J (on the main floor past Life Labs). You will be provided with some introductory forms that you will need to complete regarding your medical history. After completing these forms, you will be seated in an examination room where a medical staff will record your weight, height and blood pressure as well as complete your vision examination. A urine sample will be requested following this for required testing. The physician will then meet with you and review your past medical history. A detailed physical exam in accordance with the Ministry of Transportation requirements will then be conducted. Upon completion of the exam, you will be provided with your completed forms.
Due to the varying complexity of each individual patient, please set aside 30-60 minutes for your visit.

What is the cost?
Please contact the office to obtain rates. Discounted rates for the full medical review, physical examination, and completion of paperwork if booked >24 hours in advance. Same day service is also available. Prepayment will be required before the service is rendered.
Additional paperwork required from the Ministry of Transport after the completion of the original examination will be charged extra.
