Glazier Medical Centre is committed to protecting the privacy of our patients and the confidentiality and security of all personal health informaiton with which we are entrusted by our patients. All transactions involving the use of personally identifiable patient information must be collected, used and stored in a confidential and secure manner, while being available to authorized users for pateint care, administration, education, research and other authorized purposes.
Glazier Medical Centre has a Privacy Policy which outlines our commitment to collecting, using and disclosing personal health information. Our privacy commitment is based on the following 10 principles.
Glazier Medical Centre has identified an Information Privacy Officer who is responsible for managing privacy and confidentiality issues within the clinic.
Personal information is collected within the clinic only for certain, specified purposes. Data is not collected for other purposes unless it is explained and consent is obtained from the patient.
Patients have the right to determine how their personal health information is used and disclosed. For most health care purposes, consent is implied as a result of the patient's consent to treatment; however, in some circumstances express consent may be required. If express consent is sought by Glazier Medical Centre, a patient may choose not to give consent. If consent is given, a pateint may withdraw consent at any time, but the withdrawal cannot be retrospective.
Collection of personal health information is limited to the information required for the purposes identified.
Personal health information is only used, disclosed and retained for the purposes identified except where required by law or with the consent of the patient. Personal Information is not kept longer than is required for the purposes identified.
Glazier Medical Centre will take reasonable steps to ensure that information is as accurate, complete and up to date as necessary to minimize the possibility that inappropriate information may be used to make a decision about the individual. Limitations on the accuracy and completeness of personal health information disclosed will be clearly set out to the recipient where possible.
Glazier Medical Centre has numerous safeguards in place to ensure that personal health information is protected and only individuals who need to know the information to perform their work duties access personal information. Safeguards are technical, organizational and procedural. Personnel are required to sign a confidentiality agreement.
A Statement of Information Practices is available to the public that provides a general description of the Clinic's information practices, describes how to contact the Information Privacy Officer, describes how an individual may obtain access to request correction of a record and descrbes how to make a complaint to the Centre and to the Privacy Commissioner of Ontario.
Patients have the right to request access to Glazier Medical Centre’s record of their care. If an individual believes that the information contained in the record is inaccurate, changes may be requested.
